What Is Absolute Eosinophil Count (AEC)?

Absolute Eosinophil Count (AEC) is the estimated number of eosinophils circulating in the blood. Unlike the eosinophil percentage alone, AEC provides an absolute measurement of eosinophils and can be useful when evaluating eosinophilia, allergic disorders, parasitic infections, inflammatory conditions, and selected hematological disorders.

AEC = WBC × Eosinophils % ÷ 100

When WBC is reported as ×10³/µL, the calculated AEC is obtained in ×10³/µL. Multiplying the result by 1,000 converts it to cells/µL.

Example AEC Calculation

Suppose a patient has a total WBC count of 8.0 ×10³/µL and an eosinophil percentage of 5%.

AEC = 8.0 × 5 ÷ 100

AEC = 0.40 ×10³/µL
AEC = 400 cells/µL

What Does a High AEC Mean?

AEC combines the total WBC count with the eosinophil percentage to estimate the actual circulating eosinophil count. This makes the absolute count more useful for assessing the degree of eosinophilia.

AEC and Complete Blood Count

AEC is derived from parameters commonly available in a Complete Blood Count (CBC) with differential. The calculation requires:

  • Total WBC count
  • Eosinophil percentage

Automated hematology analyzers commonly report both the percentage and, depending on the analyzer configuration, an absolute eosinophil count. A calculated AEC can be useful for laboratory and educational reference when an absolute count is not directly reported.

Clinical Significance of Eosinophilia
