What Is Absolute Neutrophil Count (ANC)?

Absolute Neutrophil Count (ANC) is an estimate of the number of neutrophils circulating in the blood. ANC is commonly used to assess neutropenia and to provide clinical information about the patient's neutrophil count.

Unlike the neutrophil percentage alone, ANC takes the total white blood cell count into account. This makes ANC particularly useful when evaluating patients with low or high WBC counts.

ANC = WBC × (Neutrophils % + Bands %) ÷ 100

When WBC is entered as ×10³/µL, the calculated ANC is obtained in ×10³/µL. Multiplying by 1,000 converts the result into cells/µL.

ANC Calculation Example

Suppose a patient has:

  • WBC = 6.5 ×10³/µL
  • Neutrophils = 60%
  • Bands = 5%
ANC = 6.5 × (60 + 5) ÷ 100

ANC = 4.225 ×10³/µL

ANC ≈ 4,225 cells/µL

Why Is ANC Important?

Neutrophils are an important component of the innate immune system and help protect the body against bacterial and fungal infections.

A reduced ANC is referred to as neutropenia. The degree and duration of neutropenia are important when assessing potential infection risk.

ANC vs Neutrophil Percentage

Neutrophil percentage represents the proportion of white blood cells that are neutrophils. ANC estimates the absolute number of circulating neutrophils.

For example, a patient may have a relatively normal neutrophil percentage but still have a low ANC if the total WBC count is substantially reduced.

ANC and Band Neutrophils

Band neutrophils are immature neutrophil forms. When bands are separately reported, their percentage may be added to the segmented neutrophil percentage for the traditional ANC calculation.

How to Calculate ANC

  1. Obtain the total WBC count.
  2. Obtain the neutrophil percentage from the differential count.
  3. Obtain the band neutrophil percentage if separately reported.
  4. Add the neutrophil and band percentages.
  5. Multiply the WBC count by the combined percentage divided by 100.
  6. Convert ×10³/µL to cells/µL by multiplying by 1,000.

When Band Neutrophils Are Not Reported

Modern automated hematology analyzers may report neutrophil percentages without separately reporting bands. In such cases, the band component may be entered as 0% when using this calculator.
