What Is the Reticulocyte Production Index?
The Reticulocyte Production Index (RPI) is a calculated index used to assess the adequacy of bone marrow red blood cell production in patients with anemia. It adjusts the reticulocyte percentage for the patient's hematocrit and the prolonged maturation time of reticulocytes in the circulation.
RPI Formula
Corrected Reticulocyte Count = Reticulocyte % × (Patient Hct ÷ Normal Hct)
RPI = Corrected Reticulocyte Count ÷ Maturation Factor
The RPI provides a better assessment of marrow response than the uncorrected reticulocyte percentage alone, particularly in patients with significant anemia.

RPI interpretation should always be considered together with the degree and cause of anemia, clinical findings, hemoglobin concentration, iron status, vitamin status, renal function, and other laboratory results.
Clinical Use of RPI
RPI can help differentiate anemia caused primarily by reduced red blood cell production from anemia associated with increased red blood cell loss or destruction.
Low or Inadequate RPI
A low RPI in an anemic patient may suggest inadequate bone marrow production. Possible causes include iron deficiency, vitamin B12 or folate deficiency, bone marrow disorders, chronic disease, and reduced erythropoietin activity.
Higher RPI
An increased RPI may indicate an appropriate marrow response, such as after acute blood loss or during hemolysis, provided that the marrow has the necessary substrates and functional capacity.
RPI vs Reticulocyte Count
The reticulocyte percentage alone can appear elevated simply because the percentage of circulating reticulocytes increases when the total red blood cell population is reduced. RPI corrects for the degree of anemia and reticulocyte maturation, making it more useful for assessing marrow response in anemic patients.
